Identification of the material that was removed or disabled, along with its prior location (preferably the URL).
A statement under penalty of perjury that you believe the removal or disabling was a mistake or misidentification.
Your name, address, phone number, and email address.
A statement that you consent to the jurisdiction of a Federal District Court for your location, or if outside the United States, the Federal District Court where Workway is located, and that you agree to accept service of process from the party who submitted the original complaint.
